Do I need a re-entry permit for my non-immigrant 'O' multi-entry visa when leaving Thailand?

I’m on a non immigrant “0” visa multi entry.

I am leaving Thailand in a few days. Do I need to get a re-entry permit? It was issued 2 months ago.

Nope. If it's a multiple entry visa you dont need a re-entry permit.

Only single entry/extension of stay requires a reentry permit

If it’s a multiple entry visa (and still within validity date) then there is no need for a re-entry permit. Please do check it is a multiple entry. Those visas exist but are rare.

I’m flying out in 2 weeks and plan to get a re-entry permit at the airport. Should I take anything with me, forms, photos? Thanks

You don’t have to but if you take a filled out form TM8 and a passport photo it might take off a few minutes of processing time.
