My brother and I had same issues. Says to upload documents we already did. Called Chicago embassy and was told that we didn't upload. Watched documents upload again. My brother received ok and I'm still waiting.

What visa you selected? normaly when they rejected they said something for what the point why rejected

Did you remember to click the little upload button below the thumbnail after you dragged and dropped your docs into the fields? It's a 2 step process and sometimes people miss that you have to press upload on your previewed image for it to be actually recorded.
Wild! When I did it, I thought I needed the visa before the COE so I only sent in my visa application and they emailed me back saying they can't complete my visa until they got a COE application. The embassy website for LA says the COE application requires a valid visa though.. so good luck to everyone
when I applied, I had to submit the COE application and visa application at the same time. I did not have an approved visa yet when I submitted for the COE
