What is everyone attaching in further consideration when going in on a visa exempt?
I keep getting an error to attach, but have no clue what to attach... I have everything else...

For the pre-approval, I attached my insurance and passport. Once pre-approved, I added in my ASQ hotel booking and flights details. It might vary for different embassies though.

Strange, to me it only asked for 3 documents: flight, ASQ and insurance. For the preapproval just the passport if I remember correctly.
I did book the flight and even the hotel but nobody ever asked me for that (except for the hotel address, but not the actual confirmation, even on arrival)
yeah, it varies across countries. I was asked for all this before they would proceed with my application. Even got rejected once because I did not submit the documents for my outgoing flight.

I presume you're talking about COE? Maybe you started wrongly? A few people accidentally started as if applying with a visa.
