What are the current forms, copies, and documents we need to bring to immigration to apply for a 30 day extension (following the 60 day TR visa)?

yes. They take pictures of you with their camera on the desk, and they take a copy of your passport.90 days report is the same thing, just bring your passport. Very easy ๐Ÿ™‚

, i agree with Benjamin's list. You may also need to prepare a photocopy of all the stamped/marked pages in your passport (not just the one with the Thai immigration stamp). Nonthaburi Immigration usually asks for this too.
