TLDR : Answer Summary
For a 30-day visa extension in Phuket, you'll typically need to provide: TM30 proof of residence, a filled out TM7 application, copies of your passport data page, visa (if applicable), entry stamp, TM6 departure card, a passport-sized photo, and a fee of 1900 baht.
