What documents are required to extend a 60-day tourist visa in Chiang Mai?

Going to immigration in Chiang Mai next week to extend my 60 day tourist visa for tourism purposes. Has anyone been recently? What docs are required? So far I have found form TM.7 which I fill out when I arrive, photo 4x6 cm, original passport, copy of passport and relevant pages, and a photocopy of departure card. I don't have a departure card, never received one, maybe I was sleeping on the plane when they were passed out. Is it required? Any advice appreciated. Thank you.

I believe Chiang Mai also requires you to have a copy of TM30 for tourist extension.

They stopped handing out departure cards July 1st so if you entered after that you didn't get one

You can apply for the extension at either the main location by the airport OR the location on the 2nd floor of Central Festival Mall

This is a general idea of what you need for a 30 day extension based on tourism
