What documents do I need to apply for a 30-day extension of stay in Thailand, and is my address verification sufficient?

I'll be going into Jomtien Immigration to apply for a 30 day extension of stay on my visa exempt arrival 6 days ago on a US passport and have the following to show:

1. TM7 Extension of Temporary Stay form -Must include the complete address of your hotel or apartment

1. A copy of your passport ID page

1. A copy of your entry stamp or a copy of your visa, if you entered with one.

1. A copy of your Thailand departure card or TM.6

1. Thai visa extension photo requirements:- 1 passport photo, 4x6cm

1. 1,900฿ fee for extension paid in cash

A.) Am I missing anything?

B.) With regard to #1 above. I went with her to Jomtien immigration to report my address on day 2 which is the condo we live in, and they stapled a piece of paper in the back with the name of the condo in it and an official red stamp. She brought the contract, her ID, etcetera because everything is in her name (contract, all bills and so forth, nothing in mine). Is that paper all I will need to verify my address and that I live there, in order to get the extension?
