What are the requirements for filing a TM30 and do I need a photo and map for my new house in Thailand?

I'm renting a new house and am heading to immigration Friday (closed tomorrow) and had a few questions: Do I need to take the photo in front of the house again and do I need to hand draw the map? Do I need a TS28 or 30? Thanks in advance.

These are the things you need in Phuket to file a TM.30

Even though it says "residence certificate" believe me those are the TM.30 requirements for Phuket
