TLDR : Answer Summary
A UK national arriving in Thailand under the 45-day visa exemption can initially apply for a 30-day extension. However, after that, they might face a denied extension if they seek to extend further. The recommended option is to perform a border run to re-enter Thailand and obtain another 45-day exemption. Alternatively, acquiring a tourist visa before departure would have been beneficial, allowing for an initial stay of 60 days, extendable by 30 more days.

UK now uses the e-visa system so applications are processed quickly.

Why not get a tourist visa before leaving the UK? That would give him 60 days which can be extended by a further 30 giving a total of 90 days and no need for a border run

Yes, your friend can apply for 30 days tourism extension for one time only. Then possibly a 7-days-application-denied stamp if he applies for another extension after that. That's all he can get inside Thailand on visa-exempt entry. The easiest would be border-run when his 45 days stamp is up, as you say.
