Why not get a tourist visa before leaving the UK? That would give him 60 days which can be extended by a further 30 giving a total of 90 days and no need for a border run

Yes, your friend can apply for 30 days tourism extension for one time only. Then possibly a 7-days-application-denied stamp if he applies for another extension after that. That's all he can get inside Thailand on visa-exempt entry. The easiest would be border-run when his 45 days stamp is up, as you say.
