Have done maybe 6 border bumps at Mai sai with METV and never been asked for anything . But I always care have the 20,000 on me just in case .

Okay, you're highly unlikely to be asked for proof of onward travel stamping into the country on an METV or at the immigration office if you're getting a 30 day extension on a 60 day entry on an METV.

You mean when you land in Thailand? Theoretically you need 3 proofs (20k baht, accommodation and onward ticket). In practice it's rare that they ask, especially if this is your first METV. However, they can ask and if you don't have and of the 3 proofs they can deny you entry

No, extensions they do not ask for onward travel ticket - but having said that, if for any reasons they have concerns about a person working illegally or anything else etc they can ask and do what they want - and reject the extension by providing get out of Thailand 7 day stamp - but rare!

No you don't generally. That is if you already have one. In the application process you may have to show some tickets...
