in this case if you test positive they only pay your medical treatments, you have to pay for the hospital quarantine by yourself

No need a special insurance tailored especially for Covid, but you'll need a medical insurance that does NOT exclude Covid. So yes, Covid coverage still needed.
Immigration didn't check anything insurance-related for me when I came in around a week or two ago, but I'm a non-O. Tourist/retirement/etc may be different dependent on visa requirements
