Is it difficult to obtain a Thai Pass if staying in different hotels on Days 1 and 5?
Is it hard to get a Thai Pass if Day 1 and Day 5 is in separate hotels? We want to do Day 1-3 in phuket and Day 4 onwards in Chiang Mai. Or should we stick to the same hotel the whole Day 1 to Day 5.

For the Test and go, you only need to stay for 1 night at a hotel. Day 5 you can go to a clinic and get tested and submit your forms.

It's now more difficult to get insurance that the government likes. I've been rejected for the insurance I bought in Korea.

no requirements , you just need to have it booked so you can apply and have 2 confirmed hotels showing day 1 and day 5,,,

You'll need to book your SHA+ hotel in Phuket and your 1st Covid test through the PSAS website, and then book your day 5 test and hotel stay with an SHA Extra Plus hotel in Chiang Mai. You'll need the second booking to say that it includes a Covid test and is fully paid.
Then you'll need to send all of these documents in as part of your Thailand Pass application.
