Is it hard to get a Thai Pass if Day 1 and Day 5 is in separate hotels? We want to do Day 1-3 in phuket and Day 4 onwards in Chiang Mai. Or should we stick to the same hotel the whole Day 1 to Day 5.

Not difficult, just more work.
You'll need to book your SHA+ hotel in Phuket and your 1st Covid test through the PSAS website, and then book your day 5 test and hotel stay with an SHA Extra Plus hotel in Chiang Mai. You'll need the second booking to say that it includes a Covid test and is fully paid.
Then you'll need to send all of these documents in as part of your Thailand Pass application.
