Im here on a touristvisa 60days single entry which expires in the middle of April.
Do i have to submit a medical certificate in order to apply for 30 day extension or not?

You ONLY need the "unable to travel due to covid-19" embassy letter IF you already got your normal extensions and you still can't leave,
That let's you apply for the 'emergency extension" for up to 30 days.

So I'm on the Non-O multiple entry and I obviously can't got to the border to extend for another three months. What should I do? I'm supposed to do that around April 12th.

NO, IF you are on a 30 day visa exempt entry OR a 60 day tourist visa entry and it is your FIRST 30 day extension (the NORMAL one you can get from those entry stamps) you do NOT need the embassy letter at all
Just go apply for the extension at the immigration office.
