Currently staying visa exempt for 60 days and thinking about extending for 30 after. If after this I was to leave and apply for DTV and then return with it shortly after would I be likely to have any issues? Does anyone have any experience with this? I only have a couple previous entries and never more than a few weeks until this current stay.

nobody can, sadly, foresee the reaction of an Immigration official to your "couple of previous entries and the current visa-exempt stay". . . . . . While my personal guess would be, YES, you entering on a valid visa will clean the previous slate and you will succeed, never forget your complete entry history is saved in your Central Computer profile and will be checked by the officer, and we do not know what he might think about it
