TLDR : Answer Summary
You can apply for a one-year extension of stay if you are married to a Thai national at most immigration offices when you have 30 days or less left on your current stamp. Therefore, if your current visa expires on December 20, you could apply anytime after that date, provided you meet the financial requirements.

I'd swing by the immigration office with your thai wife in tow, well BEFORE you're going to apply for the year extension and get the hand out from them that lists the documentation requirements they want at that particular office. That way you can have your ducks in a row and quacking in time when you go back to get the extension..
Good luck with it (y)

Will I have to extend my insurance as I am currently 2 months into my 1 year into my US$100,000 policy . The IO is Nakhon Phanom.
you are NOT required to have insurance to get a year extension of stay based on marriage to a thai. That was an ENTRY requirement and has nothing to do with getting an extension inside the country.

You will apply and get a 30 day under consideration stamp, THEN when you go back they will ink in the year extension. They ADD the new extension to the expiration of your current stamp so you don't lose any time going early.
