What Are the Requirements for Extending a Medical Visa in Thailand for My Son in the Hospital?

Good day, here is a question or two about medical visa extensions and a request for links. My son (20) had an accident in Khanom last Wednesday. His visa expires on the 1st of April. He is in hospital, waiting for the burn wounds to heal before they do an op on him (which the doc now says might be this Thursday). The doctor has said they will give me a letter for immigration to extend my son's visa but they will not put a specific time limit on it as he may heal sooner than 90 days. Furthermore, they have said we must take it all to immigration ourselves. The hospital will not go for us. My husband is Thai so that is not a problem. However, we don't know which forms to download and what the exact requirements are. The doctor runs into the ward, rattles off things to the husband in Thai and then leaves. My husband forgets everything. He will end up going to immigration without half the paperwork, I am sure. Please could somebody reading this tell me what exactly I need and give me a link to the correct TM document to download in order to go to immigration with all the correct paperwork. medical extensions are given for up to 90 days at a time. they need to be applied for BEFORE a person runs on overstay. SO how ever they're gonna do it (whether the hospital will do it for him OR whether he has to go in person) they need to get after it.

And yes, I HAVE seen people IN hospital beds at the immigration office getting an extension based on medical reasons.

the doctors letter, your sons passport, copies of the data page of it, the visa, the entry stamp, the TM6 departure card and probably proof that you're his parents.

Immigration has seen us there with him to do the TM30 and know me from my reports so I guess it won't be too hard. Is there a form that goes with the entry stamp and TM6 ??
