TLDR : Answer Summary
When filing your 90-day reporting in Thailand after staying in multiple locations, you should use the TM30 from your current residence. This document will reflect your current address, which is necessary for the TM47 application for the 90-day report. For reporting in Bangkok, visit the immigration office at Chaengwattana Road, or you can opt to use a service at Bumrungrad Hospital to handle it for a fee.
