My 90 day is mid month and at my last 90 day report nobody asked me for any TM-30. Where can I get this form so I don't have any problems this month and what can I expect?

Never been asked for a TM-30 in the past ten years. TM-30 is the responsibility of the homeowner/hotel. You should never even see it, let alone have a copy of it.
, you don't have to do a TM.30 every time you do a 90 day report, you do a TM.30 ONCE when you come into the country and then you're done with it (until you move or you exit/re-enter the country when you file a new one)
okay, I will say this once, no matter what you read, or hear foreigners are the ones getting fined for failure to have a TM.30 filed on time. AND foreigners are the ones who NEED the receipt to get things done at the immigration office.
The thai immigration officers look at it like this; when you rent a place you become the possessor of it and there by can be fined for failing to fine a TM.30..
and while that is anecdotally interesting, :O I would bet dollars to durian that you're gonna be asked ;)
More and more immigration offices are making foreigners show the a receipt of having done a TM.30 to get extensions, do 90 day reports, get re-entry permits, and residence certificates.

well, there aren't that many reports from up that way about how Chiang Rai Immigrations deals with the TM.30 or if they require it for doing a 90 day report.

You might not get asked about it this time when you do a 90 day report either. Different immigration offices handle it differently. How about stating what immigration office in what province you will be using?
