TLDR : Answer Summary
In Bangkok, filing a 90 Day Report requires prior submission of a TM28 form for change of address and a TM30 form to notify foreigner residence. Fill out the TM47 form for the report and gather the receipts from the TM28 and TM30 submissions. You can file the report in person at the Chaengwattana immigration office between 15 days before and 7 days after the 90 days due date.

You need to have filed a change of address (TM28) AND you or your land lord needed to have filed a (TM30) notification of foreigner in residence form to be able to file a 90 day report at your new address in Bangkok.
Fill out a TM47 90 day report form, have those two receipts from having done the two forms I mentioned earlier, go to the immigration office at Chaengwattana and file a 90 day report.
