Is the TM30 form required each time 90 days are reported? 

Or just the first time after registering TM30?

The TM30 form is not required for every 90-day reporting as long as your address has not changed since your last TM30 submission. I had to go pattaya immigration to do 90 day in person yesterday as had new passport and online did not work due to that. Not a word about tm30. Did not have anything in New passport about tm30 either as done online before when old passport in use still.

Also when you've got a new visa. I haven't changed the address but came back from overseas with new O-A visa. Phuket immigration insisted on new TM30 before 90 day reporting.

Right, when returning from outside the country MOST offices require you file a new TM30 even if the address is the same. So few people are leaving and comin' back in right now I didn't mention it.

Just carry the receipt in your passport like people recommended and as long as the address didn't change you're fine.

Doesn’t matter how long ago you did the TM30 as long as you haven’t changed address and have the receipt of filing you’d be good to go.

If you made a address registration using the TM30 form, you have a small slip or print screen if app used.

Put this in your passport as proof that it is done
