I am purchasing Thai health insurance to comply with O-A extension of stay. Question: what start date should I choose in that the EoS renewal date is November 3 but I am going to immigration on October 20 or so?

TLDR : Answer Summary
When purchasing Thai health insurance for your O-A extension, ensure the policy starts before your immigration appointment on October 20, especially since your extension expires on November 3. It's recommended to have coverage from the date you visit immigration to avoid any gaps in coverage. Some insurance companies offer 13-month policies that allow for flexible start dates, but if you choose a policy that only lasts one year, starting it too close to your extension's expiration could result in losing part of your extension period.

Meines Wissens gibt es keine O-A Verlängeruing, nur eine Verlängerung als "Rentner-Visum".

Oder hat sich da was geändert?

Make sure you are covered by the policy on the date you GO to the immigration office to apply for your extension. Unfortunately I have not found any insurance company on the list of the 14 companies that will pro-rate a policy (OR that will take more than one year of premiums either) so that means you get a year policy and you can only select the date it starts. :/ IF you start it 2 weeks before your extension expires it means you will not get a full year extension (they will only give you an extension for as long as your policy is good for so if you start it 2 weeks before your current stamp expires you'll lose 2 weeks on the extension you get)

thank you for your advise. I will start insurance on 1 nov. Go for my extension on 2 nov. See what happens.

Do we now need insurance for a non-o one year marriage visa? Thanks.

Some (if not all) insurance companies will give you 13 months on the OA insurance (one month priced pro-rata), so you have flexibility to have the insurance run from the date of application through to the expiry date of your visa or previous extension.

I wasn't offered that by Pacific Cross where I did mine. I asked for the date to be the same date as the expiry of my existing extension so I could get the full 12 months on the new extension even though I was at immigration 8 days prior to the expiry date. I was at immigration on June 18th and my current extension expired on June 26th. I had no issue with this at Jomtien. No doubt other immigration offices could take a different view.
