I got it at chaengwattana; im assuming i have to go there? Are there TM 47 forms there or do i need to print it?

Where did you get your extension? Usually they staple a paper into your passport stating the date you do your first 90 day report. You can go in person from 15 days before down until 7 days after that due date to file a report.

You fill out a TM.47 (90 day report form) go to the immigration office and turn it in. IN Bangkok all you need is the filled out TM.47 form. Other immigration offices want copies of your passport.
