Do I need to fill out any forms for the 90-day report at the drive-through in Phuket Town?

First time trying to use the drive through in Phuket Town for 90 day report. Question - I am bringing photocopies of passport (visa/personal info page) and my departure card. But do I need to fill out any forms? If so, it seems it’s not a drive through if I don’t have the forms before getting there. Is this the gist?

I'm sometimes asked to fill in TM.47 but you can do 90 day report online

Drive through should only require your passport. They may request proof of filing a TM30, which your landlord or hotel should be able to provide.

my passport had the departure card and TM30 report already stapled inside. Just handed that over and they added a third slip of paper (90 day report) clearly stamped with the date I need to report. The drive thru was a compete game changer for me. There was a line outside the door (as most days) but this was under 10 minutes without getting out of the car.

you should try the new online 90 day report. But the drive-through is an excellent alternative in phuket.

just trying to work out if they are requiring a TM30 or not. My guess is the thing stapled in to your passport is actually the departure card called a TM6. They usually take the arrival half on entry and staple the other into your passport. If you check the card it should say TM6

I cant speak for Phuket but in CM they put the TM30 slip with paperclip at the back of your passport along with the 90 day report.

most people have their TM6 (departure card) stapled in to their passport on entry into Thailand. In Phuket they put the 90 day receipt slip in the back of the passport- the one that tells you when your next report is due. As a proof of a TM30 filing can be a multitude of things (the system doesn’t give a receipt, so many use a screenshot of the website, or phone app they used) this generally isn’t put in the passport. You are correct that different offices do things in different ways but I’m guessing that that slip of paper in the back of your passport is the TM47 90 day receipt telling you when you next need to do one.
