Hi. What documents do I need to bring to the ministry of labor and then to the immigration to cancel my work permit and visa by myself? (both of them are valid until second half of August). The company is under closure in the meantime, does it make any problem for me?

The accountant said that she can cancel my work permit, but can not go to the Immigration, because the company is already under closure and it doesn't have other staff anymore. It seems I won't be able to do this alone without the termination letter, what options do I have now?
