First, they no longer issue these at Changwattana. You must now go to Muang Thong Thani. I found this out the hard way. :)
And do not research the internet (as I did) for the list of requirements for this certificate. Here's what you need:
Fill out a simple form that they give you there.
Bring your Passport
Bring signed copies of:
- picture page of passport
- copy of last arrival page
- copy of last extension page
- copy of TM.6
- copy of 90 day receipt sheet
- 200 baht
They will send the certificate of residency to your home in about 2 weeks.

AND
just in case people don't know IF you're not on a visa/extension where you do 90 day reporting (and have done at least one) you can't get a certificate of residence from Bangkok Immigrations. ;)

Thx. In the end it was quite easy. I made the mistake of following some info on various sites such as copies of rental contracts, passport photos, etc. It was all nonsense. I should have come to this reliable site first. :)
