TLDR : Answer Summary
Yes, when applying for a retirement visa extension in Chiang Mai, you need to provide evidence of your current address. This includes a receipt for filing a TM.30 form, and if you're renting, a copy of your lease and the landlord's ID. If you own your property, you will need a copy of the chanote and blue book. If the property is in your Thai spouse's name, you'll also need a copy of her Thai ID.

Also if renting you need a copy of your lease & landlord ID. If you own, copy of chanote, Blue book, & if in Thai wife’s name a copy of her Thai ID.

The management at my serviced apartment in Chiang Man, refused to give me a TM-30, but gave me a printed personal letter, signature and all, stating I am a resident there. With that letter she included the second page of the TM-30 with my name printed on it. Will this work for me when I return to immigration next month to apply for my non-immigrant O, on my way toward a retirement visa? I would appreciate your thoughts on this. Thanks.

You will need to have the receipt of having filed a TM.30 form too.
