I was at Korat IO today for a 30 day extension based on tourist I had a Copy of : Evisa, passport page, Visa Page, TM30, TM7, and Photo.
They also asked for my Wife's ID card House Book which I though was strange.

I just did my 30 day extension in Nong Bua Lum Phu. I handed them my paperwork and first question was who are you staying with? About then my wife walked in and handed over a copy of the house book and her ID. Probably took me ten minutes and I was out the door, fast service.
I guess she is the owner or rentor of the place where you stay and she has to make the address registration. Just bring the form to register is not enough.
