What are the requirements for extending a 60-day tourist exemption in Thailand by 30 days?

We are on a 60 Day tourist exemption and are looking to get an additional 30 Day Extension. Apart from TM47 form, TM30 and our passport do we need to provide anything else?

THESE are the common found requirements for the 30 days extension of stay at your local Thai Immigration office for 1900 Thai baht.

Be sure to have a TM30 address registration on file with Thai Immigration.
