TLDR : Answer Summary
To apply for a multi-entry visa in Thailand based on a work permit, the required documents include: copies of the Non-B visa, extension of stay, the most recent entry stamp to Thailand, passport data page, a filled-out TM8 application for re-entry, and a passport-sized photo.
