What do I put as my arrival date on the TM.8 form for a re-entry permit to Thailand?

Hi guys filling in a TM.8 form.

To take to Suvarnahumbi airport to get my re-entry before leaving for the UK.

It might sound silly but the question on page two is:

I Arrived in Thailand on

Date Month Year.

So is this my last time I entered Thailand. Which was 10th May 2018 when I visited Vietnam?

And I take this form a photo and ฿1000 along with my passport to desk just after security but before passport control in the left hand corner of the passport control room. TYIA
