To get to the re-entry permit office from the priority lane, you pass immigration after they stamp your passport. Walk down the ramp and thru the duty free shops and back up a ramp to the main immigration lanes. The office is in the corner and is small with only enough room for two officers.
I prepared the following in advance: completed TM-8 form, color photo, photocopy of passport main page and last entry stamp. The officer took these items and 3800 baht. I waited 10 minutes and she returned with my passport with a re-entry permit and a receipt.
