Cash only. They charge a little extra for filling in the form and photo. Tell the IO you need a permit and they will direct you to the desk. I've never seen it busy

I did it at Siracha immigration a few weeks ago. 1000 baht and pretty painless. When leaving bkk for Singapore on the 13th I was happy I already had the stamp as it looked like about 25 people in line for the stamp at the immigration booth area. 1200 baht at bkk immigration. Sometimes I've heard the line is short. Arriving back on the 15th with my tourist VISA went well. GL

Jim Duggan Re-entry stamps in Thailand cost the same in base fees at both immigration offices and airports (1,000 THB single / 3,800 THB multiple). However, getting the stamp at the airport can cost more if you require photo services or form assistance, which may incur an additional 200 THB fee. I was wrong, had it around the wrong way!

Yeah, easy. Down in the corner, right before you go through immigration. They’ll even fill in the form for you for an extra 200 baht (if you don’t want to or have not downloaded it prior). It’s 1900, or 2100 if they do it (I think) and you’ll need passport pics. It takes less than 10 mins.

Jack Peters to answer your question,no you won't get a 30 day extension for a second time, buy a 90 day visa this is why Thailand is tightening up on extensions

Alex Ve I’m asking that if I was coming back to the uk for 3 months after being there 90 days then would it not be possible to do another 90 days?? In effect I would be in Thailand for 180 days per year and 180 days in the uk.

Paweł Chruściel You’re clearly stuck in a rigid, Soviet mindset if you think my answer is useless. It is offered as an option and was also directed at the author of the question. If he wasn’t able to do it at the airport, where else would he do it and would he require further questions? I’ve seen people posting similar questions when they’ve just done their extension but not a simultaneous re-entry permit. Yes, it does happen.
