I have left Chiang Mai to Phuket.

Nearest immigration office is at Blue Tree Immagration Thalang, I am told.I have had one 30 day extention this year and for this I supplied the address of my friend there. Can you remind of what I need this time for my second extenion, including reference from the hotel i am staying.Thanks!

Extension of a tourist entry? If so you are only allowed one 30 day extension per entry stamp. You must go outside the country to get another entry stamp, or visa, or change your reason to stay inside the country such as ED visa.

no only 1 per entry.

You can enter on visa-exempt twice per calendar year by land, no limit by air. You can apply for 30 days tourism extension ONCE per entry on visa-exempt or tourist visa.

then you can get one 30 day extension of stay at Thai immigration office in Phuket.

Make sure you have a TM30 on file when you apply. Here's the common found requirements for the 30 days extension

Hmmmm... IF you already have had One 30 day extension of stay on your current Arrival stamp .. there's NO second 30 Day extension of stay available to you.
