TLDR : Answer Summary
The discussion focuses on applying for an extension of stay rather than a specific retirement visa. Key requirements include copying only certain pages of the passport, providing a Non-O visa and current entry stamp, and filing a TM.30 form. Additional documents may include a lease agreement. Following approval of the extension, a re-entry permit is needed for future travel.

In ChiangMai, take your papers to the copy shop downstairs and tell then what you are applying for, they will know what pages to copy. And it’s cheap, 2 baht/page

You don't need every page of your passport copied. The immigration office tells people to do that because no one seems to know what pages really need to be copied.

There is no personal information form anymore.:/ That was discontinued. However in Chiang Mai they do want to see the receipt of you having filed a TM.30 (alien in residence ;) ) form.

For copies you need

data page of your passport

Non-O visa

current entry stamp

TM.6 departure card

You might also need to show your lease agreement.

No police report or medical certificate is required when getting an extension of stay.

Keep in mind you are not changing any status and you are NOT getting a "retirement visa" <- that's because there is no such thing.

You are applying for a yearly extension of stay based on being over 50 (retirement).

Don't forget once you get your yearly extension IF you're going to exit/re-enter thailand in the next year you'll need a re-entry permit. (Either a single for 1000baht or a multiple for 3800baht).
