TLDR : Answer Summary
To extend a 30-day visa exempt stamp in Phuket, you generally need to provide a passport, fill out form TM7, supply copies of your passport data page and entry stamp, provide a passport photo, the TM6 departure card copy (if issued), and pay a fee of 1,900 baht. This can be done at immigration offices in Phuket Town, Patong, or Blue Tree water park.

This is a general idea of what can be required. It is NOT office specific, so post on a Phuket group or take this website from the Phuket Immigration Volunteers as a guide
