What documents do I need to extend my 30-day visa exemption in Thailand and do I need to provide an address?

Hello everyone. I'm Looking to extend my original 30 day visa exemption for another 30 days. Just confirming I have the necessary documents:

- Passport 


- Passport size photo


- Photo copy of passport (make sure have copied main page and page with visa exemption stamp)


- Departure card


- Onward flight ticket (is this needed)


- 1,900THB cash (credit card is not accepted, correct?)

Is that it? Do I need to provide any address in Thailand?
