TLDR : Answer Summary
Your brother, currently on a 60-day multiple entry tourist visa in Pattaya, can apply for a 30-day extension at the Jomtien Thai Immigration office. This extension is only available once. If he requires more time after this, he will need to exit Thailand and perform a 'border bounce' to re-enter, where he can obtain another 30-day stamp on arrival. Alternatively, he could apply for a new 60-day tourist visa from a neighboring country, but this may be more complicated. Consider his disability when choosing the best option for traveling to the border.

As above or 60 day tourist visa from embassay in neighbouring country , but that would seem extra hassle for what's needed.

Border bounce from Pattaya if his disability allows him to sit in a minibus for hours or go via the airport if that's more comfortable for him.

Don't fly back the same day though. So that might actually be harder work for him.

He can get ONE 30 DAY EXTENSION OF STAY at the Jomtien Thai Immigration office. ONLY one time.

So if he is going to stay longer, he must leave Thailand and reenter by doing a BORDER BOUNCE and get a 30 day STAMP on arrival.

That can be extended for ADDED 30 days One time.

It can be done the same day with an experienced border run service company in the PATTAYA area.

But be careful with multiple times entering Thailand in a year. Especially if Thai Immigration Officers are seeing you as not a tourist anymore.
