At Phuket immigration, needed copies of passport Bio page, visa stamp page, TM30, a recent passport size photo, our rental lease. Cost us 500thb per certificate of residency needed, we got 3, bank account, driver's license and to purchase a car, so 1500thb in total

But don’t be surprised if they will not give it to you until you have stayed at the same adress for 3 months, since you are not a resident until then.

The cost and speed depends upon the Immigration Office your TM30 is filed at. I got my residence certificate in Sa Kaeo province free in about 25 minutes (it took that long as the front office person I was dealing with had never seen an online TM30 before).

Depending on the office, no charge is a week, or if they offer rush service for a fee 3-500 baht the next day
