How do I correctly fill out the TM.8 form for my non-O visa extension in Thailand?

Good morning,

I am filling a TM.8 form as I am flying abroad on 13th April. I came on a non o visa in October and have an extension to January 2027, this was processed in Chon Buri immigration office. On the back of TM.8 it asks, after ticking Non-Immigrant Visa box, issued at and date. So can I assume that this is Chon-Buri on the date I received my extension? I know I can do this at the airport but I have to do my 90 day report this week, so thought i'd do this while I'm there.

Most likely, the docket is a reminder for your first 90 days report. I had that, ignored it as I left Thai before 90 days.
