Have the TM7 forms changed since last October, and what are the required documents for an extension application at Cheang Wattana?

On my last visit to Cheang Wattana last October to update my TM 30, I collected a new set of TM7 forms. Does anyone know if these forms have been amended since then, or have they stayed the same. Also, I’m shortly going to apply for my annual extension using the deposit of 65,000 Baht per month. Has anyone recently done their extension at CW using this method and has a list of all items required to be submitted with your application.

The way you can tell if the TM7 form is the correct one or not is to look on the back of it next to the place where you affix your photo to it

IF there is a price for the cost of the extensions it is the OLD form. If it has no price it is the new form ;)

The new TM7 does not have the price of the extension printed on it. If your copy does it’s the old one.

Don’t have list for CW but this page lists what Phuket needs. It won’t be too dissimilar.
