Does anyone have the 60 day extension form for Chiang Mai. I think it's the tm7 form but I know Chiang Mai has their own specific form that has the office on the top right-hand side. Anybody have a PDF so I can print it

Update from Chiang Mai Immigration

I successfully got a 60 day extension due to covid today at Chiang Mai. No under consideration period, fully stamped in to the end of January. I entered Thailand on a Non-O visa for marriage, and already used my 60 day visit family extension which was expiring on Nov 26.

This is what I brought with me

Printed email showing that the Canadian embassy won't issue extension letters

Covid extension request that Tod Daniels posted in this group, but they made me redo this form on an all english version

TM30 receipt

STM2

Acknowledgement of Penalties for Visa Overstay

Passport photocopies for name page, all entry stamps and visas, and TM6

Photos

It took less than an hour from start to finish, no problems whatsoever. Hope this helps

 do you know if Corey Rose went to Promenada or the Immigration near the airport?

Last time we went to Promenada early this month they told us we needed to go to the Immigration office near the Airport for this.

 yeah I saw this and apparently since I got one 60-day extension already Chiang Mai doesn't give a second one out even though other offices do

 promenade 2nd floor only the one office so go early if you can

hopefully someone may have the Chiang Mai forms used.

But be aware even ChMai may have changed recently.

Best to visit them and pick up the needed forms.
