I'm a US Citizen, but already living abroad (with my wife, three kids). Do I have to go back to the US to apply? What's the relationship between applying online to current/physical location?

You apply for a visa to Thailand at the Thai embassy that services where you physically are. So if the Thai embassy in the country you are in has adopted e-visa then you apply through that embassy online. If they have not adopted e-visa then you apply in person. Check the website of that Thai embassy to see their process and requirements.
