I am registered as living at my house in Chiang Mai and recently signed a 6 month lease for a house rental in Phuket. I have 2 residences that I maintain. Do I need to report my address in Phuket when I stay there and re-report my address when I return back to Chiang Mai?

TLDR : Answer Summary
When maintaining multiple residences in Thailand, such as a house in Chiang Mai and a rental in Phuket, you are theoretically required to report your address in Phuket to the authorities, especially if you need to interact with immigration. However, in practice, if you do not need to deal with immigration in Phuket, it may not be necessary to report your address when staying there.

In theory yes. In practice probably not if you do not need any interaction with immigration in Phuket.

I had a friend who has 2 residences. 1 in phuket 1 udon Thani. His previous 90 report was in Udon but came to phuket for 2 months work. His next 90 day came due and he put Phuket address. They rejected his report as didnt match immigrations records.

In short keep reporting already registered address unless you move permanently or feel like going to immigration every 3 months.
