Do I need to provide previous addresses when filling out a TM30 for my current place of residence in Thailand?

Hello, I was wondering if, when filling out the paperwork for a tm30, do I only need to provide the current place of residence? I was at a hotel, then I was renting a condo for 1 month, and am now going to be returning to the hotel. I am going to be applying, again, for a covid extension.

Also, should I provide paperwork showing dates encompassing both residences?

a TM30 is filed to show where you reside NOW. So don't worry about anything before that.

I'd take

's advice and let the hotel file the TM30 online for you and just have them print the screen shot that they did it. That's all you need for proof

Keep it simple... Wait for the move to the hotel and ask them for Their screen pic of their filing the TM30 as a guest in the hotel...

Then proceed with your planned application with Thai immigration.
