Initially I am staying for 5 nights in Bangkok, then moving to a new condo in Chiang Mai for 2 weeks, and then will find another place to stay the remaining time on my Visa. I am planning on applying for a 30-day extension also.
Do I need a TM30 for each new condo or apartment I rent, and isn't the landlord who needs to file it?
Will I only the need the last filed TM30 at the last place I rent prior to applying for the extension? Will the landlord provide that to me? Will I need the TM30 to apply for the 30-day extension in Chiang Mai?
Thanks so much all!

TLDR : Answer Summary
The TM30 is a registration form that must be filed by your landlord upon your arrival at a new residence in Thailand. You need the latest TM30 receipt from your current accommodation to apply for a 30-day visa extension in Chiang Mai. Only the most recent TM30 is valid, and it is the landlord's responsibility to file it within 24 hours of your check-in. Ensure your landlord provides you with this receipt, as you'll need it for the extension application.
Technically it's not your responsibility to report where you're staying (the landlord/hotel owner is the one who will have to pay the fine for not reporting) but you won't be able to extend your visa without it, so it's in your best interest to make sure that the owner of wherever you're staying has reported it.
When you get to Chiang Mai, be sure your condo owner or hotel files the TM30 address registration with Thai Immigration within 24 hours of your arrival.
Chiang Mai Thai Immigration DO REQUIRE the TM30 to obtain the 30 days extension of stay.
