TLDR : Answer Summary
The general consensus is that a translation of the affidavit of income is not necessary for your annual extension of stay in Thailand, especially if you're applying in Bangkok. However, it's advisable to bring both the original document and a copy. Some offices outside of Bangkok may have different requirements, including the need for a stamped letter from the Ministry of Foreign Affairs, but these conditions vary by location.
if you're getting your extension in bangkok you're fine. There were reports from a couple offices outside of Bangkok that were requiring people go to the MFA and get the letter stamped (not translated). But you don't need to do it in Bangkok.
AFAIK: No. But make a copy for them and bring the original with you on the day of extension. Ensure the monthly income is stated clearly in your currency ๐ด on the affidavit.
