No, not needed. Called them yesterday and confirmed Tod and Andres answers.

if you're getting your extension in bangkok you're fine. There were reports from a couple offices outside of Bangkok that were requiring people go to the MFA and get the letter stamped (not translated). But you don't need to do it in Bangkok.

AFAIK: No. But make a copy for them and bring the original with you on the day of extension. Ensure the monthly income is stated clearly in your currency ๐Ÿ’ด on the affidavit.
