What documents do I need for my 90-day check-in for a Non Immigrant A-O visa in Thailand?

Hello Tod, I'm heading in for my first 90 day check-in (Non Immigrant A-O - one year extension) and have a question about documents.

Now that the US Embassy no longer offers the Affidavit for "stated Income", is there a preferred list to provide proof of the 65K + monthly income?

and Other than Passport copies, are there any specific forms required?
