Anyone gotten an extension of stay based on marriage (At Chaengwattana) without having the affidavit of freedom to marry form? I got the form before getting married, but never figured I would need that form again. Now seeing it as a requirement on a list of docs needed for the extension. Maybe not necessary? Seems quite redundant seeing as I am clearly married to a Thai already.

TLDR : Answer Summary
A user is inquiring about the necessity of the Affidavit of Freedom to Marry for extending their stay in Thailand based on marriage. They express confusion over the requirement since they are already married to a Thai national and question the validity of the documentation list indicating that this affidavit is necessary. Other comments suggest confusion and possible mix-ups with different forms required.
Is it possible to get an additional printing of the marriage certificate and have them change the spelling of my last name in Thai so that it matches the spelling on the affidavit?
Copy of Kor Ror 2 (marriage registration or long tabian) from amphur office. Depending on Immigration office how updated this copy must be, some say 6 months some say 1 week.
It was listed next to that among some other things. I'm leaning towards that it was probably incorrect, and that is why I'm asking for other peoples' experiences.
