Under a US passport you have the same requirements as any other American. You can enter on just your valid Thai ID card, if you have one.

Yes not sure about insurance.
If you are on a Thai Passport you need insurance coverage only for quarantine period.
Non Thai passport you normally need insurance to cover you for length of visa.
Prayut is making an announcement in twenty minutes, speculation until then, also depends on Bangkok governor who is resisting calls to allow bangkok to open up until 70% of citizens are vaccinated.
Have to see later but remember Prayut is the man who stood in front of the UN in 2014 and promised to halve deaths on Thailands roads by 2020

And if you enter Thailand on your US passport, you will be treated as a foreigner in that you will require a valid visa and you willo be subject to 90 dayy reports, etc.
