TLDR : Answer Summary
To enter Thailand from the USA, you need a Certificate of Entry from a Thai embassy or consulate. This requires approved COVID medical insurance and a confirmed hotel reservation—15 nights in an ASQ hotel if arriving in Bangkok, or 14 nights in a SHA+ hotel for Phuket (or a 7+7 option). COVID insurance is mandatory.

Yes, you would need covid insurance. There are 2 ways to enter Thailand, through state quarantine or through one of the sandboxes. Here are some FAQ's for the Phuket Sandbox.

Everyone needs a certificate of entry from a Thai embassy/consulate. To get that you need medical insurance specifically for Covid and a confirmed hotel reservation. If flying to Bangkok then a 15 night booking in a ASQ hotel, if Phuket then 14 nights in a SHA+ hotel (or 7 + 7)
